Your checklist will provide a central tool, but it will need to be a part of a resource that offers more substantial understanding of what it offers, how to use it, and how to respond. You have done a lot of this work in Ch. 3, so you can use that, revised for the teacher-user, as part of the project. 
**I don't see anything about parent contact and communication about abilities and needs in the checklist, and this is a major focus of your statement of problem and literature review. How can you include communication and collaboration in the tool?
Here's what I recommend:



1) A compelling title, something like 'Providing Individualized Supports for Inclusion.' 

2) An intro section, explaining a) what is inclusion? b) importance of understanding individual student abilities and needs, c) importance of parent-teacher-student communication in providing individualized supports, d) how to use the tool, e) the checklist, f) explanation of each of the categories in the checklist, with some suggested resources (diagrams, assessment tools, strategies, etc.) for achieving each of these checklist criteria.
