For your course project, you will be creating a Personalized Patient Treatment Plan for a hypothetical patient utilizing the multidisciplinary approach. You will use the patient scenario presented on the Course Project Introduction page located in Module 01 for your hypothetical patient.

This week you will submit Part 1 - Outcomes, Quality and Approach of your course project. After reading the patient scenario, complete the following:

Part 1 - Outcomes, Quality and Approach

Approach Plan: Identify which disciplines to include on the team.

Include why you think these disciplines should be included on the team and what contributions they will make to the patient treatment plan.
Make sure to think about the patient as a whole and evaluate the need to include nutritional and spiritual members for example.
Quality of Life and Desired Outcomes: Based upon Marjorie's desire to live and improve her lifestyle. Develop a plan to work with her on setting attainable goals/outcomes.

Identify the specific patient goals
Example: Patient wants to lose weight to be more active and social
Education Plan: Based upon the disciplines that are included on the team, develop an education plan that meets Marjorie's needs.

Identify information that will enable the patient to meet each of her specific goals
Examples: Nutrition, exercise, and social aspects of the patient's life
