For your course project, you will be creating a Wellness Plan for a hypothetical patient/client (Joe/Jane Patient). You will choose 1 of the top 3 most common health conditions in the U.S. today to afflict your "fake patient" with and you will tailor the Wellness Plan to improve their current condition. 
This section of your project has 2 parts; the creation of your patient profile and a summary of your chosen health condition.
Part 1 - Create a Patient Profile 
For part 1 of this course project section, you will select the health condition of the hypothetical patient (Joe/Jane Patient) who you will be working to help. Create your Patient Profile for Joe/Jane and be sure to afflict them with 1 of the top 3 most common health conditions in the U.S. today.
The patient profile should contain a brief description of the patient that would usually be obtained from an initial patient interview (fake interview with your Joe/Jane Doe Patient) and include the following information:
Demographic information - area of residence, type of neighborhood, rural vs. urban, etc. (environment)
Level of physical activity overview
Dietary overview
Lifestyle overview
Social activity overview
Mental Health status overview (subjective)
Current patient overall health - how are they feeling? (subjective information)
Pain scale level (subjective - scale 1-10, with 1 being no pain and 10 being severe pain)
Quality of life score (subjective - scale 1-10, 1 being no care to live and 10 being blissfully content)
Using your critical thinking skills, compile your information and write a summary of your patient's current health status based on your patient profile, identify any red flags, and/or situations that may be contributing to the patient's health status.
Part 2 - Summary of Chosen Health Condition 
For this part 2 of this course project section, you will need to research and summarize the health condition that Joe/Jane Patient has been afflicted with. Include the following in regards to the disease process. Be sure to cite your sources.
Name of the condition and explain why you chose it.
What population/age group is typically affected by this disease?
What are the signs and symptoms?
Identify 3 or more Complementary and Alternative Medicine (CAMs) methods to supplement, treat, and prevent the disease you identified.
