DQ1: 

My PICOT statement as related to population health problem. Non-vaccination in ESRD patient causes an increase in hospitalization during influenza season, increase in administering vaccination rate for influenza and pneumococcal vaccine by comparing previous two years’ hospitalization rates that includes admissions related to influenza and pneumonia complications which will help decreasing the overall hospitalization and missed treatment rate secondary to influenza/ pneumonia adverse effects in a two-year flu season period. This supports the population of focus by preventing admissions to the hospital and missed dialysis treatment. The mortality rate in ESRD patients from pulmonary infections is 14- to 16-fold higher than in the general population, it is alarming that there has been a 31% rise in infection-related hospitalization rates since 1994 in this population (Soni et al. 2013). The problem that I have selected is amendable to research based intervention using the PICOT format because this format will help answer the questions proposed by providing possible therapies or interventions.


DQ2

The PICOT I developed in NUR-550 is based on the adolescent population, a decrease in the stigma towards mental health patients through education will ultimately lower suicide rates within 6 months to a year. The change initiative I am proposing is increasing education about adolescent suicide related to anxiety and depression both in terms of treatment options and the reality of this disorder. For both adults and adolescents, a very real stigma exists that has created a barrier for people to seek help. Varying from adults though, is the additive stressors that adolescents deal with separately from anxiety and depression that they have.

By focusing on adolescents, it allows me to specifically address the unique time in their life and the varying barriers they face. I believe in the power of education and that knowledge is power. Adolescents should have every resource and every ounce of knowledge we have to offer on how to get through these times and be able to thrive.

According to the World Health Organization, depression is the fourth leading cause of illness in ages 15-19, anxiety is the sixth for 10-14 year old’s, ninth for ages 15-19, and globally suicide is the third leading cause of death for this age population. This topic is a very prominent problem that many people are afraid to talk about, which is why studying research-based interventions is vital to the health of our community. Creating a study based on adolescents has the benefit of easy access through schools, therefore being able to track success. The other reason this is a good topic to investigate is that it is an under-researched topic in both development and mental health literature (Heary et al., 2017). While this creates initial barriers, it makes for research that matters and has the potential to truly demonstration usable community interventions.

    III.    Honest Appraisal

Ahimsa, the first of the yamas, the very foundation of all yoga practice, means the absence of 
harm, the practice of compassion, kindness, and gentleness. To practice yoga is to work to bring 
this ideal into our lives, both on and off our mats, in order to contribute to greater peace and 
harmony within ourselves, our relationships, and our society. We begin, where we are, by 
bringing our awareness to our thoughts, our speech, and our actions. 
All of our actions have repercussions—from the food we choose to eat, the businesses and

ripple out in myriad ways not immediately perceptible. 
To practice ahimsa is to take the lens of mindfulness to our lives, to begin to see these ripples of

thoughts, because these are the source of our actions, and often the very root of both conscious 
and unconscious harming behaviors.
This assignment is an opportunity to treat your life like an experiment, and “try on” the practice of

the Right Wolf” by Pema Chödrön (chapter one of her book Taking the Leap) will give you a 
sense for how to approach the practice of ahimsa. Reading 1, “What is Yoga” by Donna Farhi is 
worth rereading—please focus particularly on her description of ahimsa. 
Once you’ve read, take some time to contemplate, feel, consider, and yes, practice (and, if you 
want to, research**) this concept of ahimsa, and write honestly about your insights, in the form of 
a personal essay. Your paper should be approximately 2-4 pages.
** Don’t bother with internet research; this is not a research paper, but a personal essay. 
To help you generate ideas on how to approach this assignment, you might wish to read the 
supplemental readings in Canvas. Chapter 3 of Bringing Yoga to Life, by Donna Farhi, discusses 
the yamas and niyamas (the first two of the Eight Limbs). Pages 29-37 are of particular 
relevance to this assignment, and I strongly encourage you to read these pages. For another 
perspective, Judith Lasater’s Living Your Yoga includes a brief personal essay on ahimsa (p.135-
140). If you use these or any other outside sources (not including the two required readings in 
Canvas), please include a bibliography listing the sources you used. 
The most important aspect of this assignment is to practice ahimsa: incorporate the idea 
of ahimsa into your daily life over the course of three or more weeks, and reflect on your 
experiences. 
Notice for yourself where and when in your life you find an attitude of ahimsa easy to maintain, 
and when more challenging. What relevance does ahimsa have for you in the way you make 
your livelihood, in your social relations, your personal health, and your inner thoughts and 
attitudes? Is ahimsa relevant to your yoga practice?
As you incorporate ahimsa into your life, think about how you feel it makes sense to apply it. Do 
you practice ahimsa with yourself? With other people? Toward animals, or nature? Is ahimsa 
relevant to your life? If so, how can we translate the ideal of nonharming into daily practice? 
How does an appreciation for the interconnectedness of all life come to bear on our
understanding of ahimsa? What relevance does ahimsa have at the personal level, the relational 
level, the societal, and the global levels?
Think about historical figures like Mahatma Gandhi, Martin Luther King Jr. and Mother Teresa, 
and other forces for peace in the world. Consider the ways our food is produced, its ecological 
and ethical impacts on the environment and on animals. How does our consumption contribute 
(or not) to ahimsa? Fashion, politics, partying, friendships, recycling—how do these and other 
elements of your life relate to the idea of ahimsa?
For those interested in their food choices, there are many great books that discuss the harmful 
impacts of industrial food production. The Face on Your Plate by Jeffrey Moussaieff Mason is 
one that provides a perspective that is firmly (some might say radically) grounded in ahimsa. 
If you are interested in both political and personal alternatives to violence and suffering, The 
Search for a Nonviolent Future by Michael Nagler is a great overview of the broader implications 
and applications of ahimsa.

            There are different approaches to health that are practiced all over the world.  Conventional health is considered mainstream health and has been proven effective based on many research studies.  Complimentary medicine uses other treatment methods such as acupuncture to manage side effects of mainstream treatment of cancer (National Institutes of Health, 2018). Health approaches that incorporate complementary health and holistic health in combination with mainstream approaches is considered integrative health (National Institutes of Health, 2018).
            A medical approach that is used instead of conventional treatment is considered alternative care (National Institutes of Health, 2018). A non-mainstream practice that combines conventional medicine is, complementary care. A non-mainstream practice that is used instead of conventional care is, alternative care (National Institutes of Health, 2018). Integrative care goes much deeper.  It provides a holistic, patient-centered approach to well-being by including spiritual, emotional, mental, and social aspects of health instead of just treating one organ or symptom (National Institutes of Health, 2018). Complimentary Alternative Medicine (CAM) is divided into five categories (Institute of Medicine, 2005). The first category is the alternative medical system which includes Chinese medicine, homeopathy, ayurvedic and naturopathy. Mind-body is a category of CAM that involves mental healing, prayer, and meditation. Biological based CAM uses herbs like Gingko biloba and St. Johns warts, non-herb products like fish oil and special diets and functional foods. Manipulative and body-based treatments consist of chiropractic care, and massage therapy. The last category is energy-based therapies that used energy, Reiki and therapeutic touch (Institute of Medicine, 2005).
            I am personally drawn to integrated health.  I belief in a holistic approach to medicine.  In my experience people are so different in how they respond to various treatment, medication and therapies.  I have seen where addressing nutritional and hormonal imbalances has corrected depression. I feel, it is important to discover what will be the most effective approach to wellness for a patient’s based on their physical, emotional, psychological, biological, functional and spiritual needs as a whole. I believe there is a place for conventional medicine and complimentary medicine.  I will use my daughter as an example.  She had a history of seizures.  Her anti-seizure medications were not managing her seizure disorder. She was still having on an average of 3 per month.  We added Cannabidiol CBD oil to her treatment plan which proved to be very effective in her case.  She was able to reduce the amount of medications she was on and has had only one seizure in the past 3 years since she started CBD. That being said this was done in a very controlled situation and a great deal of research was evaluated before making the decision.
            I believe there are many ways to achieve health while minimizing adverse reactions.  I would like to see more non-conventional treatments, if used carefully and wisely, incorporated into more practices.
