Common barriers to EBP implementation include inadequate knowledge and skills, weak beliefs about the value of EBP, poor attitudes toward EBP, lack of EBP mentors, social and organizational influences, and economic restrictions (Melnyk, Fineout-Overholt). Inadequate knowledge and skills is an obstacle that no one is comfortable with admitting. Most often as a leader you so not know what knowledge and skills staff lacks until an issue arise and you have to question why a task was performed or education was given in a particular way. For example, a patient presented to dialysis on the last treatment day of the week with a clotted access. The RN involved completed the access referral request faxed information to vascular center and sent patient home with instruction to await a call from access center on Monday. Patient daughter called the unit on the following Monday to inform us that patient expired over the weekend. After investigation the patient missed the 2nd treatment of the week and 3rd due to clotted access. RN never called to discuss clotted access with nephrologist and to inform last time patient received treatment. The process should have been for RN to call and consult MD with present scenario and await orders. No education was provided to the patient that if he felt ill, short of breath or chest pain to go to the nearest emergency room immediately. This is an issue that need address so another peer would not make the same mistake

Evidence-based practice (EBP) is essential for nurses because research shows that nursing practice that is based on evidence improves patient outcomes (Mathieson, Grande & Luker, 2018). Although nurses recognize the importance of utilizing evidence, studies have shown there are various obstacles and barriers they encounter that can hinder the implementation of EBP. One of the most common barriers reported by nurses is limited time and knowledge of EBP (Mathieson, Grande & Luker, 2018).

Nurses repeatedly report that lack of time created by issues such as staffing shortages and heavy workloads are significant barriers to researching and using EBP (Mathieson, Grande & Luker, 2018). Lack of time also creates difficulties for implementing new EBPs or making practice changes based on new evidence. This is further compounded by the fact that there is a vast amount of evidence to review because medicine is constantly evolving which makes it very challenging, if not impossible, to keep up with all of the latest research. Additionally, many nurses have limited access, knowledge or skill in researching evidence (Mathieson, Grande & Luker, 2018). They may be unfamiliar with how to efficiently and effectively use research databases or have limited access to reputable databases because of factors such as cost or technology restrictions.

Background 
The setting is an 82-bed hospital located in a small city. One day an employee of the maintenance department asked the supervisor, George Mann, for an hour or two off to take care of some personal business. Mann agreed, and he asked the employee to stop at the garden equipment dealership and buy several small lawnmower parts that the department required. While transacting business at a local bank, the employee was seen by Sally Carter, the supervisor of both human resources and payroll, who was in the bank on hospital business. Carter asked the employee what he was doing there and was told the visit was personal. On returning to the hospital, Sally Carter examined the employee’s time card. The employee had not punched out to indicate when he had left the hospital. Carter noted the time the employee returned, and after the normal working day she marked the card to indicate an absence of 2 hours on personal business. Carter advised the chief executive officer (CEO), Jane Arnold, of what she had done, citing a long-standing policy (in their dusty, and some would say infrequently used, policy manual) requiring an employee to punch out when leaving the premises on personal business. The CEO agreed with Sally Carter’s action. Carter advised Mann of the action and stated that the employee would not be paid for the 2 hours he was gone. Mann was angry. He said he had told the employee not to punch out because he had asked him to pick up some parts on his trip; however, he conceded that the employee’s personal business was probably the greater part of the trip. Carter replied that Mann had no business doing what he had done and that it was his—Mann’s—poor management that had caused the employee to suffer. Mann appealed to the CEO to reopen the matter based on his claim that there was an important side to the story that she had not yet heard. Jane Arnold agreed to hear both managers state their position.

My area of research would involve evaluating and establishing the critical point areas of intervention in emergency trauma care following accidents. Most fatalities and associated complications following road accidents occur because the medical intervention is not well aware and equipped with the right knowledge and capability to identify and address the critical point areas of trauma that need care (Terrier et al., 2017). Consequently, the research will seek to evaluate and understand the precise areas that are common and prevalently in need of trauma care to save and preserve lives. There is scanty research on the subject and lack of prevalent attention on the actual areas where critical intervention, if well directed would help to save lives (Ton et al., 2019). Subsequently, the focus of this research will evaluate actual cases and develop a guiding template that would potentially help to intervene and improve strategic efforts of intervention following the occurrence of such incidents.

I settled for this topic because there is scanty research on the subject, yet accidents account for high levels of fatality and loss. The narrow moment after an accident and first response following the event plays a critical role in defining and influencing the status of the victim. However, when this window is not utilized effectively, this often results in many cases of fatality and deaths (Thao et al., 2018). Consequently, there is an opportunity to save lives and enhance the process of intervention to a point where it can identify and closely meet the needs of patients during instances of emergency care. Ultimately, I also seek to make a viable contribution to the efforts of intervention and prevention against many of the fatalities during accidents. The study will add to the existing few studies on the subject and contribute that would effectively save lives.

Highlights from a retrospective study using an expert panel review methodology indicates that "Potentially preventable or preventable deaths represented 7% of cases that had attempted resuscitation from paramedics; and system of care was considered suboptimal thus providing opportunities to improve trauma systems" (Beck et al, 2019). Hence there are indeed opportunities to improve emergency and trauma care in our population.

In 1975 R. Adams Cowley stated in an article that "the first hour after injury will largely determine a critically injured person's chances for survival" (Rogers et al, 2014). This is widely known as the golden hour and is considered most important time to provide quality care for a patient that received traumatic injury. Hence, in light of this findings from research in this area can be useful to save lives and improve emergency care provided to patients.

Once an accident occurs, the clock begins ticking. There is a narrow window to assess injury and treat said injuries. When you establish these critical care points will you establish those points with in field treatment, treatment in route, and treatment in ER care? This is a very interesting topic of research, and if you were to focus on different points in which care should be done, such as with in field care, you could expand this research to include education. This could include education for bystanders, and emergency responders. This is a great topic.
